Insurance information
I am currently an in-network provider with United/UMR. The service covered by insurance is 60 minute therapy. Your payment will be less than what is listed above because it is based on the insurance company’s negotiated rate and your benefits plan. You are responsibile for determining your benefits and for paying any portion that is not covered by insurance.
If you have a different insurance carrier, but would still like to use insurance coverage you can determine if you are eligible for out-of-network benefits. To use this option you pay the full rate for the services and I will provide documentation (“superbill”) of your use and payment so that you may seek reimbursement from your insurance company.
If you have Medicare, I can not bill for out-of-network services and must ask you to sign my opt-out form.
Please take into consideration that any medical claims must contain a mental health diagnosis which is then included in your medical record and may impact future coverage for medical insurance, as well as other types of insurance, such as life insurance.
Other payment options
If you are in need of services but your personal budget cannot cover the full fee, I do offer a limited number of reduced rate spots in my caseload. The availability varies, please inquire.
I also offer services through some EAP plans and REPS.
Additional information
I meet with clients in person in my office at 1041 Lincoln Ave (0ld Pilot Building, Suite 200) in Steamboat Springs, CO. When needed, arrangements can be made for remote therapy by phone or video.
I only serve clients who are residents of Colorado.
